Please bring your insurance card with you for appointments in the office and notify the staff of any changes, i.e. Insurance coverage, address or name changes, and employment. We will do our best to answer your questions and file all claims for you.
We are a contracted provider with most of the commercial insurance companies. We are a Medicare provider. Medicare plan typically allows for preventative care (Yearly visits with Pap Smears) to be covered only for visits every other year unless you take medication in which the Provider’s prescribe or have/had a problem. Medicaid is accepted for maternity only for new patients. If you are an established patient we will continue to provide you with gynecological care. We are a contracted provider on Amerigroup, Caresource, and Molina plans. Please contact our insurance clerk if you have questions regarding how your claims are being paid.
We are a contracted provider of many insurance companies. We have provided you with a list of insurance company’s websites. This list is by no means a list of all the insurance plans that we are contracted with, only the most frequently used companies. Please check with your insurance company to make sure that we are covered by your plan. You can do this by calling the customer service number located on your card or by clicking on one of the following links. On some of these websites you can also check your benefits and claims that have been submitted.

FEES
We ask that you check with your insurance company to be sure that we are a contracted provider with your insurance plan. We require co pays to be paid at the time of service and we will file all your claims to the insurance company that you have given to us. If there is any additional amount that you owe, we will balance bill you after your insurance company pays.
We will call all maternity patients insurance companies to find out what their benefits are for maternity. If there is an amount that you are responsible for, you will receive a letter setting up a payment plan for you. We ask that your responsible amount be paid in full by your 36th week of pregnancy. Self-pay maternity patients will be required to pay $800.00 at your first office visit. The remainder of the charges will be set up in a payment plan for you. We ask that the remaining amount be paid in full by your 36th week of pregnancy.
Surgery patients will be informed of the amount they may be responsible for prior to their surgery. Patients having elective procedures are responsible for payment prior to their surgery date.
