All patients are invited to call with medical-related questions during regular office hours. Our trained office staff will be happy to respond to your inquiries. However, it would be unfair to our other patients for the doctor to leave the exam room to accept every phone call. Non-urgent calls will be returned within 48 hours. Urgent calls will be handled as needed. Please make sure you are available when the provider returns your call and provide an updated contact number when leaving a message.
REGISTRATION FORMS
All new patients are required to fill out a registration packet prior to their first visit. You may download, print, fill out, and bring in the following documents. You can also request, time permitting, for us to mail them to you if you provide us with your address at the time you make your appointment. You may also opt to arrive 10 minutes early to your first appointment in order to fill out the required paperwork.

OFFICE ETIQUETTE
No smoking is allowed. Tobacco smoke is hazardous to your own health, as well as the health of others. If you need to smoke, please step outside of the building to do so.
Please refrain from wearing any fragances to your office appointment due to allergy sensitivities experienced by the providers.
No food or drink is permitted. We make every effort to keep our waiting room clean and comfortable. This is impossible if we allow food or drink.
Please be courteous to our staff and fellow patients by limiting your cell phone calls while in the waiting room. Please turn cell phones off when you go back to see your provider.
Be aware that there could be as many as five providers working at the same time. You may not be called back in the order of your signing in; other providers will be seeing patients also. Click on the Providers link to learn more about our Doctor’s and Midwives!
If you have been waiting over 30 minutes, please ask the front desk if there is a problem. We will try to notify you if there is a delay in your appointment.
ULTRASOUND INFORMATION
In our continuing effort to provide you with the best medical care, our doctors have provided you with the opportunity to have ultrasounds done here in our office. Here are the things you need to know before you arrive for your ultrasound. For Gynecological Patients: * There is no longer the need to drink large quantities of water prior to your ultrasound. * Most gynecological ultrasounds are performed vaginally. For Maternity Patients: * Ultrasounds are usually performed vaginally until your 15th week of gestation. After this time they are performed abdominally. * Ultrasounds are performed for medical reasons only. If the technician is able to determine the sex of your baby you will be informed if you desire to know. However, ultrasounds are not done solely to determine the gender of your baby. * Ultrasounds are not included in the global charge for your maternity, but will be filed to your insurance company.
You may bring two guests (children included) of your choice in for the ultrasound. Our rooms are small and too many people in the room distract the technician.
